Running out of the hospital in Genshin Impact is a hidden achievement available in the desert area of Sumeru. To add this achievement to your collection, you have to run through the underground cave of the desert, along the way getting acquainted with the story of the escaped patient Abbas.

How to get through the cave
Go northwest from the village of Aaru to the area of Dara al-shifa. Focus on an abandoned hospital surrounded by small groups of Hermits. On the right side of the building behind the broken fence you will see a shallow hole and a dendrocule.
Stones blocking the path are easily broken with a single falling hit or multiple auto attacks. After descending down, a short cut-scene will start, after which you can start exploring the cave.
How to find the exit
Initially, you will find yourself in a temporary camp of an escaped patient. Break the stone mound behind you to advance.
Pretty soon you will come to a larger part of the cave, in the center of which a group of hilichurls and stone-shelled lavachurls are guarding a precious chest. To advance along the route, you need to remove another stone mound on the opposite side; if you wish, you can bypass the enemies and head straight for it.
You will soon see the guarded chest in the middle of the fork again. The passages to the left and right are littered with stones. Behind the right pile of stones is a small branch of the cave with the Dendroculus, and the left road leads to the exit from the underground.
If you turn left, then another mountain of cobblestones awaits you behind the stone barrier. Break it to trigger the second cutscene and expose the surface.
How to get the “Running from the hospital” achievement
During the cut-scene, you will see a hoe stuck in the ground. After interacting with her and reading a short description, you will immediately receive this achievement from the “Wonders of the World” section
Inscriptions left by an escaped patient
If you want to learn more about the history of Abass, you can spend a little more time exploring the cave and pay attention to the wall inscriptions.
The first inscription is on the right side of the precious chest. The peculiarity of wall notes is that the text is not visible from afar, and only a faint shine attracts attention to the walls.
In these notes, the patient talks about his acquaintances, about the evil doctor, exhausting him and other patients, and prays defender of the desert to complete the trials, allowing him to return to his homeland.
The second inscription is located in the same part of the cave as the first one, near the previously stoned passage. Here Abbas tries to cheer himself up and tells himself to keep digging to get out of the cave.
The third inscription is a little further than the second. Go to the next part of the cave and note the far wall on the right.
The fugitive continues to have a dialogue with himself; desperation is felt in his words, he writes that he accepts the punishment of the king of the desert and does not want to die.
On the way up you will find the fourth entry. It is rather short, and in it Abbas asks the sky to come down to him and talk to him in human form.
The fifth and final inscription awaits you at the exit of the cave. It is even shorter than the previous one, consisting of only three words: “All will be forgiven.”
